The general objective of this Technical Cooperation (TC) is to generate rigorous, generalizable evidence on how digital and data-driven solutions can enhance the effectiveness and equity of chronic disease management in Brazil and the region. The TC will evaluate alternative approaches to improve the enrollment, adherence, and retention of patients with non-communicable diseases--particularly hypertension and diabetes--in ongoing care programs. In particular, the project will assess how innovative follow-up and engagement models can strengthen patient continuity of care, reduce disparities in access, and inform scalable strategies for integrating artificial intelligence and adaptive communication tools into health service delivery.
